How does health insurance work in terms of payment?

Dad pays a monthly rate for the insurance. I had free health insurance through work. It had perscription coverage. I had to pay a $15-$25 co-[payment for each perscription I got. I paid the co-pay to the pharmacy when I picked up the perscription. The pharmacy then billed my insurance provider for the remainder of the cost of the perscription.
It sort of worked the same way at the doctor;’s office. I would go to the Dr. and pay a $25 co-payment to the clinic. The clinic would bill my insurance company for my visit, who would pay about 80% of my bill for that visit.
